By filling in this appointment request form you acknowledge that you are aware of (LIMA’s) Lubbock Integrative Medical Associates Appointment Cancellation Policy. We require a 24 HOUR Notice if you need to cancel a scheduled appointment.
Effective January 1, 2020 you will be charged a $45.00 fee for not showing up to your scheduled appointment or arriving late to said appointment. Late arrival may result in your appointment being rescheduled. The same fee will apply to cancellations without 24-hour notice.
